Connecting a Patient Portal to Legacy EHR Systems

The Situation

Picture a healthcare provider that wants to give patients direct access to their records, appointment scheduling, and messaging with their care team. But the actual clinical data lives in an EHR system that was never designed with a public-facing patient interface in mind. The new portal has to expose exactly the right data, in real time, without becoming a second system of record that can drift out of sync with the source of truth.

How We'd Approach It

The first step, before any interface design, would be mapping exactly what the EHR can expose. Most modern systems support HL7 or FHIR-based integration, but the depth of that support varies a lot in practice. Our custom software approach here would treat the EHR as the system of record and build the portal as a thin, well-scoped layer on top, rather than duplicating clinical data into a separate database that could fall out of sync.

Privacy and access control would be designed in from the first architecture decision, not layered on later: role-based access, full audit logging of who viewed what, and encryption both in transit and at rest. On the interface side, our UX/UI design process would involve real usability testing with actual patients, since a confusing health portal doesn't just frustrate people, it can mean they miss something important.

What an Engagement Like This Would Aim For

The measure of success in a scenario like this is less about a flashy metric and more about whether patients can actually find what they need without calling the front desk, and whether the data stays accurate and current without manual reconciliation. We're deliberately not putting a number on that here, since without a real deployment and real usage data behind it, any figure would be invented, not measured.
